All routine appointments at Bradley's Practice are booked through Zymego, our online appointment system. Zymego allows you to book, change or cancel appointments at a time that suits you, as well as join the waiting list to be notified if an earlier appointment becomes available.
If you need urgent medical care, please contact the practice by telephone. Our Care Navigation Team will ask you a few questions to understand your symptoms and ensure you are directed to the most appropriate clinician or healthcare service. Depending on your clinical need, you may be offered an Urgent appointment (within 24 hours), a Priority appointment (within 1–10 days), or a Routine appointment (within 3–6 weeks).
If you are unable to use Zymego, please contact our reception team, who will be happy to assist you.
There are several ways to request your repeat medication. You can order using the NHS Wales App, through your nominated pharmacy, by email, via eConsult, or by handing in your repeat prescription slip at reception or using the letter box outside either surgery. Please note that repeat prescription requests cannot be accepted over the telephone.
Please allow up to 72 hours for your prescription to be processed and sent to your nominated pharmacy. We recommend ordering your medication when you have around 7 days' supply remaining. If you regularly take repeat medication, you can also ask your nominated pharmacy to manage your repeat prescription requests on your behalf.
If you're due a medication or long-term condition review, you'll receive a text message from Bradley's Practice containing a secure link to Medlink.
Simply open the link on your phone or computer and complete the online questionnaire. The questions are tailored to your condition and may ask about your symptoms, medications, recent readings (such as blood pressure), or any concerns you may have.
Once submitted, your responses are reviewed by a member of our clinical team. If any further tests, appointments or changes to your medication are needed, we'll contact you. Completing your review before your appointment helps us make the most of your consultation and ensures your clinician has the information they need to provide the best possible care.
If you're unable to work due to illness, you can self-certify for the first 7 calendar days of your absence. You do not need a GP fit note during this time and should provide your employer with a self-certification form if requested.
If your illness lasts longer than 7 calendar days, you'll need to request a Fit Note (Statement of Fitness for Work). You can do this by completing an eConsult through our website or by contacting the practice. A clinician will review your request and, if appropriate, issue a Fit Note, which can be sent to you electronically or collected from the practice.
Please note that Fit Notes cannot be backdated unless there are exceptional circumstances and they are issued at the discretion of the clinician.
Once your test results have been reviewed by a clinician, you'll be contacted if any further treatment, investigations or follow-up are required.
If you have been advised to discuss your results with a clinician, please request an appointment through Zymego. If your results require urgent action, a member of our team will contact you directly.
Please allow enough time for your results to be processed before contacting the practice, as some tests can take longer than others to be reported and reviewed. Our reception team are unable to interpret or explain clinical results.
You have the right to access your medical records and other personal health information held by Bradley's Practice.
Many parts of your medical record, including your medications, allergies, vaccinations, test results and recent consultations, can be viewed through the NHS Wales App if you have online access enabled.
If you require copies of your medical records or specific information that is not available through the app, please submit a request to the practice. We may ask you to provide proof of identity before releasing your information to protect your confidentiality. Requests are processed in line with UK data protection legislation and NHS guidance.
If you live within our practice boundary, you can register as a patient with Bradley's Practice online or by completing a registration form at reception.
To register, you'll be asked to provide some basic personal details. If possible, please have your NHS number available. You may also be asked to provide proof of identity and address to help us process your registration.
Once your registration has been completed, you'll be able to access the full range of services provided by the practice, including appointments, repeat prescriptions and online services. If you're unsure whether you live within our catchment area or need help registering, our reception team will be happy to assist you.
